- Improved patient care planning & connection to self-management supports
Installation Steps:
All Accuro tools can be downloaded from the Accuro Form Cloud:
- In the Form Editor, open the Tools menu.
- Select the dropdown option Publish/Download.
- Search for “E2P” to view the complete suite of forms.
- Highlight and Download the following forms for Heart Failure:
- E2P – HF – Heart Failure Management Tool
