The Amplify Care West Regional Deployment Team has been busy supporting clinicians with onboarding for eReferral Ontario now that the eReferral onboarding pause has ended! We are ready to assist you and your team in getting set up to send and receive eReferrals with ease. Our team provides onboarding support, guidance, and training for Ocean eReferral with the goal of streamlining processes, reducing administrative burden and ensuring that you and your team feel confident with eReferral workflows.

eFax Initiative with Local Delivery Groups
We are excited to share that this spring, 832 specialists (188 Ocean Directory Listings) from London Health Sciences Centre and Hamilton Health Sciences have been onboarded to eReferral Ontario and are now available in the Ocean Healthmap! Many of these sites are accepting eFax eReferrals using new provincial standardized eReferral forms.
This work is helping make the referral process more consistent and easier to manage, while reducing paper-based workflows and supporting better communication between providers.
In 2026/27, Amplify Care will continue supporting Ontario Health West, Hamilton Health Sciences and London Health Sciences Centre to onboard more hospitals, clinics and specialists across West Region. Together, this work will support Ontario Health targets and help continue the rollout of eReferral Ontario across the region.
* Local Delivery Groups (LDGs) were established in June 2023 to serve as regional digital implementation leads for provincial and regional priorities. For more information on LDGs, please visit Ontario Health’s website.
We Want to Hear From You!
The West Deployment Team at Amplify Care will be hosting a series of eReferral webinars in the coming weeks. These webinars provide high-level demonstrations of the Ocean eReferral platform, giving participants a clear understanding of the full eReferral process.
We will begin with sessions focusing on eReferral workflows using the Ocean portal (web access). Webinars focusing on eReferral workflows with integrated Accuro, TELUS PS Suite and OSCAR Pro EMRs will follow.
These webinars are ideal for clinicians and referral management staff who want a comprehensive overview of how Ocean eReferral can streamline and enable greater efficiencies within the referral management process. eReferral Wishlist – We Want to Hear From You!
Working with clinicians to onboard to eReferral Ontario, we often hear the question, “How can we get more organizations on eReferral so my organization can send/receive a higher volume of referrals electronically?”
Please let us know who you would like to see using Ocean eReferral by either completing our brief OH eReferral Sender/Receiver Referral Form, or by emailing us at WestRegion@amplifycare.com. Our team will connect with them in an effort to support onboarding and growing the eReferral Ontario Network together!
Standardized eReferral Forms
In an effort to reduce administrative burden, improve clinician and patient experience, and to support the goals of the Patients Before Paperwork (Pb4P) initiative, Ontario Health and Amplify Care are collaborating to develop standardized eReferral forms.
As of May 31st, 29 Standardized eReferral forms are now available:

All specialty forms are developed using a standardized structure and established best practices, and are co-designed with consultant specialists, family physicians, nurse practitioners, specialized care organizations, administrators, and Ontario Health Teams.
There are currently 401 Directory Listings in the West Region using a Standardized eReferral Form*!
An open clinical review of Phase 4 Standardized eReferral forms will begin in the summer of 2026. Please visit the Amplify Care website for more information.
eReferral Webinar Series
In case you missed it…
The West Region eReferral Deployment Team at Amplify Care hosted an eReferral Webinar Series, which ran weekly from February to April, 2026. This series covered Ocean eReferral workflows for web-based Ocean users, as well as Ocean users integrated with Accuro, TELUS PS Suite and OSCAR Pro EMRs.
A wide range of questions were received throughout the sessions, and our team compiled these questions into a Frequently Asked Questions document. This document, as well as recordings of the eReferral webinars, can be found on our Amplify Care website.

Help Close the Gap in Chronic Kidney Disease Screening
Early to moderate Chronic Kidney Disease (CKD) often presents without symptoms—leading to missed opportunities for diagnosis and intervention in primary care.
At the same time, 90% of CKD cases are low risk of progression, meaning primary care clinicians are well positioned to manage the majority of these patients with the right supports in place.
Amplify Care is developing a practical, EMR-based CKD toolkit designed to support clinicians:
- Identifying at-risk patients
- Diagnosing CKD earlier
- Managing care using guideline-informed pathways
We are inviting clinics to participate in trialling the screening tools and providing feedback to enhance adoption.
