Hepatitis C (HCV) Screening and Care Tool for Telus PS Suite

The Ontario Roadmap recommends universal screening for all adults (18 and older). This tool helps you complete one-time HCV screening and care as part of routine primary care for all adults.

This toolkit has been developed by Amplify Care in collaboration with the Viral Hepatitis Care Network (VIRCAN) at UHN’s Toronto Centre for Liver Disease.
