eReferral: A Connected Care Success Story

Participants will hear firsthand about the journey of eReferral adoption, key successes achieved across Ontario, and the collaborative efforts that continue to build a more connected and coordinated healthcare system.

Who you’ll learn from
Kim Lynch
Director, eReferral Program
Kim has a long and impactful record of accomplishments built over more than 35 years in Ontario health care. Working across acute care, rehabilitation, and complex care settings, she has held diverse leadership roles spanning planning, project management, information management and technology, communications, and change management. Her experience includes leading large-scale, multi-organizational IT implementations with a strong focus on change management and adoption, as well as overseeing marketing, strategic communications, and rebranding initiatives in teaching, community, and rural hospitals, and other health care organizations.
Kim is deeply committed to collaboration and high standards of customer service, ensuring clinicians are well supported throughout periods of change. She is passionate about improving patient outcomes and strengthening relationships across the health system to advance digital health initiatives provincially and nationally.

Sarah Mian
Interim Director of Care Coordination and Access at Ontario Health
Sarah Mian is the Interim Director of Care Coordination and Access at Ontario Health, leading provincial initiatives including Central Intake, eReferral Ontario, Standardized eReferral Forms (SRFs), eConsult, and the Provincial Health Service Directory in support of the Patient Before Paperwork initiative. Over her nine years at Ontario Health, Sarah has supported major digital health and system transformation initiatives, including the Wait Times Information System, eCTAS, and COVID-19 reporting and performance management efforts. Most recently, she led the development of Ontario’s Standardized eReferral Forms and the implementation of Diagnostic Imaging Central Intake across six regions, with expansion underway into additional pathways. Sarah holds a Master of Science in Management from the University of Illinois and is a certified Project Management Professional (PMP) and Certified ScrumMaster (CSM).
