Chronic Kidney Disease Screening Toolkit

The CKD Screening Toolkit developed by Amplify Care’s EQUIP program helps primary care teams identify patients who may be due for Chronic Kidney Disease (CKD) screening and order recommended screening tests.

It supports proactive screening of patients at higher risk of CKD, including those with type 2 diabetes (T2DM) and hypertension (HTN).

Ontario Renal Network’s (ORN) KidneyWise guidelines recommend screening high-risk patients using estimated Glomerular Filtration Rate (eGFR) and urine Albumin-Creatinine Ratio (uACR).
