There’s Still Time for Clinicians to Provide Input on Standardized Referral Forms

Phase 4 Open Clinical Review is underway — feedback welcome until August 9, 2026!

Ontario clinicians still have an opportunity to help shape the future of standardized Referral forms.

As part of the ongoing Provincial Forms Standardization initiative, Ontario Health is collaborating with Amplify Care to develop standardized Referral forms that reduce administrative burden, support better referral experiences, and enable timely access to care for patients across the province.

The Phase 4 Open Clinical Review period is currently underway, and clinical feedback is being accepted until Sunday, August 9, 2026.

Clinicians are invited to review and provide input on standardized eReferral forms for the following specialty areas:

  • Diabetes Education Program
  • Ontario Lung Screening Program
  • Ophthalmology
  • Palliative Care
  • Sleep Medicine

Your feedback helps ensure these forms reflect real-world clinical workflows, capture the right information to support effective triaging, and remain practical for clinicians and patients.

Building better referrals through collaboration

Standardized Referral forms are being developed using a consistent structure and evidence-informed best practices. This approach helps ensure that forms include the necessary clinical information for appropriate triaging, without adding unnecessary complexity.

The development process has been guided by collaboration with clinicians, including specialists, family physicians, nurse practitioners, specialized care organizations, administrators, and Ontario Health Teams. Feedback gathered through each Open Clinical Review period has played an important role in refining the forms and ensuring they support the needs of those who use them every day.

Previous specialty forms have been reviewed through the same collaborative feedback process, helping establish a consistent approach across referral pathways.

Why form standardization matters

A more consistent referral experience benefits everyone involved in the care journey.

For referring clinicians, standardized forms can reduce cognitive load by creating a more familiar structure, minimizing unnecessary data entry, and supporting clearer referral completion.

For specialists and care teams, more complete and consistent referrals can help reduce back-and-forth communication and support more efficient triaging.

For patients, better referral information and improved processes can contribute to more timely updates and access to appropriate care.

This work is about more than standardizing forms. It is about creating the infrastructure needed to support high-quality, equitable, and efficient care across Ontario.
