About Charlotte
Charlotte Brubacher is a Program Manager at Amplify Care, where she has supported health system partners for more than 12 years by leading structured, sustainable OHT integrated care initiatives, program and change management for the people side of digital transformation. With more than 20 years of clinical experience as a nurse in multiple care settings, she brings a practical understanding of what clinicians need to successfully adopt digital health solutions. She supports change management specialists deploying health information technologies across multiple care settings, initiatives, and provinces, ensuring lessons learned are captured, and approaches align with Amplify-approved frameworks, best practices, and processes. She has also led East Coast teams—both on-site and remote—building national partnerships and advancing patient- and clinician-centred care through strategic collaboration and innovation.
