eReferral Ontario Now Available for Onboarding!
The wait is over — we are now officially onboarding for eReferral Ontario! The Amplify Care West Region eReferral Deployment Team is ready to support you and your team in getting set up to send and receive eReferrals with ease. Our team provides onboarding assistance, guidance, and training for Ocean eReferral with the goal of streamlining processes, reducing administrative burden and ensuring that you and your team feel confident with eReferral workflows.
Thank you to all of our partners for your patience and ongoing collaboration during the eReferral onboarding pause, during which time Ontario Health conducted a procurement process to evaluate eReferral vendors. The successful vendor, OceanMD, was selected based on their ability to meet provincial security, privacy, workflow integration and scalability standards.

What’s New With eReferral
New Standardized eReferral Forms (SRFs):
Ontario Health, in collaboration with Amplify Care, has developed standardized electronic referral (eReferral) forms with the aim of reducing administrative burden and enhancing the experience for clinicians and patients. This initiative supports the Patients Before Paperwork (Pb4P) program. These forms are designed with a consistent structure and adhere to established best practices. They have been co‑created with input from specialists, primary care clinicians, nurse practitioners, administrators, and Ontario Health Teams to ensure clarity and usability. The final eReferral forms will be available in English and French.
The Standardized eReferral Forms that have been completed so far are:

Self-Directed Onboarding with Ocean eReferral
We would like to hear from you! Ontario Health and Amplify Care are looking for clinicians and clinics to give feedback on self-directed processes, resources, and tools for eReferral onboarding. This will inform a self-directed onboarding option that could be made available to clinicians onboarding to eReferral in the future.
This self-directed onboarding approach offers clinics a more flexible and on-demand experience, allowing for digital tool adoption on your own schedule, with dedicated support only if and when needed.

Upcoming eReferral Webinar Series
The West Deployment Team at Amplify Care will be hosting a series of eReferral webinars in the coming weeks. These webinars provide high-level demonstrations of the Ocean eReferral platform, giving participants a clear understanding of the full eReferral process.
We will begin with sessions focusing on eReferral workflows using the Ocean portal (web access). Webinars focusing on eReferral workflows with integrated Accuro, TELUS PS Suite and OSCAR Pro EMRs will follow.
These webinars are ideal for clinicians and referral management staff who want a comprehensive overview of how Ocean eReferral can streamline and enable greater efficiencies within the referral management process. eReferral Wishlist – We Want to Hear From You!
The West Region eReferral Deployment Team would like to expand your eReferral network – and we need your help!
Working with clinicians to onboard to eReferral Ontario, we often hear the question, “How can we get more organizations on eReferral so my organization can send/receive a higher volume of referrals electronically?”
Please submit our eReferral Ontario Organization Request Form so we can connect with the right clinics to support onboarding and growing the eReferral Ontario network together!

eReferral Quarterly Spotlight
A new centralized referral pathway has been developed in Ocean eReferral for free exercise programs for older adults that can help prevent falls and improve overall health.
Programs are available both in‑person and virtually across Kitchener, Waterloo, Cambridge, Guelph, Wellington, Wilmot, Woolwich, Wellesley, and the Township of North Dumfries
